With the registration form, "inschrijven" you can subscribe yourself and make your first appointment. Unfortunately the form is not available in English.

 

Online registration will save you time during your first visit.

It is legally required that you are able to show us legitimation. It is therefore important that you take a valid identification document to your first appointment.
